1. Download the Risk Assessment & Appendices.
The Risk Assessment takes 1-3 hours to complete, and includes a producer interview, animals assessments, records review, and farm walk-through. Use the Pre-Assessment Checklists (pages 4-5 of the Risk Assessment) to improve your efficiency when conducting the Risk Assessment. The Risk Assessment can be done in conjunction, or separately from the NMPF FARM Program evaluation.
2. Enter Risk Assessment Scores on the Score Entry Sheet. Submit this sheet to Erin Royster at royster@umn.edu for generation of the custom report.
